Million Hearts® in Municipalities Learning Collaborative

In 2012, the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services—alongside 120 official public and private sector partners and 20 federal agencies— successfully aligned national CV disease prevention efforts around a select set of evidence-based public health and clinical goals and strategies. As a result, significant progress was made toward the aim to prevent a million CV events in five years. Million Hearts® 2022 was launched in 2017 to enhance progress and continue the achievements in reducing the burden of CVD in the United States. MH® 2022 provides a platform to promote high-impact, sustainable evidence-based strategies and serves to cultivate collaborations, identify community strengths and align synergies among diverse partners and stakeholders.
Million Hearts®2022 framework gives public and private sector partners a way to work together to improve health and prevent disease, empower patients and providers, improve and coordinate systems of care, and leverage the strengths of public and private entities. Increased use of Million Hearts® strategies has shown to build multi-sector collaborations, guide local, state, and national efforts to identify the burden of CVD, and focus implementation on high impact efforts that will bring about the most change for the community. Leveraging the experiences and resources of diverse stakeholders cultivate efficiencies across agencies and municipalities in developing their own tailored program.
NACCHO in partnership with the CDC will host a national Million Hearts® in Municipalities Collaborative to support community efforts to implement Million Hearts® strategies to reduce heart disease and stroke. Municipalities (city and county-level) participating in this new collaborative will be able to share their cardiovascular disease achievements and engage in the broader MH®2022 initiative. Through this collaborative, participants will learn and share through targeted webinars, resources, and technical assistance activities. Participants will also be able to connect with Million Hearts® national partners, subject matter experts and other municipalities doing similar work.
